First of all you will need an applicant application form which you can get from http://www.fbi.gov. You will also need to include your full mailing address, phone number, and email address. Next you will need to get the FD-258 standard fingerprint form. That can also be obtained at the FBI website as well.
You have to have this and then you will need to put your full set of fingerprints on the form. Next you will need a money order or cashier’s check for $18 dollars and be sure it is made payable to the Treasury of the United States. Finally mail all the items to the following address and remember it may take up to twelve weeks to respond.
